A Chiropractic Care Membership provides you with the best in recurring chiropractic care. Reduced rates, regular monthly services and special discounts are designed to maximize your treatment and give you predictable out-of-pocket costs as you work toward wellness. Even your family and friends can participate!

Pricing

Initial Membership Fee

The initiation fee is $149 which includes a complete chiropractic exam, treatment and 30 minute massage1.

Recurring Monthly Fee

Chiropractic membership is $49 per month and is a 12 month contract. The monthly membership fee includes one chiropractic adjustment. Any additional chiropractic visits are billed at $49 each.

All other clinic services are offered to members at a 10% discount. Simply present your active membership card at check out to receive the discount. This discount may not be combined with other offers.

Family Chiropractice Membership

A Family Add-On membership has an initial fee of $99 which includes a complete exam, treatment and 30 minute massage. The monthly fee is $49.

Policies

General Policies

  • The Center for Spine Pain memberships are non-transferable.
  • Pre-paid chiropractic treatments are non-transferable and may not be used by anyone other than the named member.
  • All membership offerings are auto-renewable. At the end of the initial term of membership, your monthly dues will automatically renew on a month-to-month basis.
  • A Family Add-On membership is defined as any immediate family member that resides in the same household. Friends or relatives are not considered immediate family. Picture ID may be required upon enrollment in a Family Add-On offering.
  • The Center for Spine Pain is not responsible for lost or stolen articles or items.
  • The Center for Spine Pain reserves the right to change clinic rules, regulations, and pricing at any time upon providing reasonable notice.

Canceling Your Membership

You may cancel your membership agreement upon the following circumstances:

  • You change your permanent residence to a location more than 25 miles from any CSP clinic.
  • Your death or permanent disability.
  • All cancellation requests must be accompanied by written proof of relocation or medical reasons (e.g. copy of driver's license, utility bill or submission of doctor's note may be required)
  • If you elect to discontinue your auto-renewable privileges after the initial term of the agreement, simply provide a 30 day written notice to discontinue your dues at your home clinic. Cancellation occurs 30 days from the receipt of the cancellation notice.

Procedures

Members are required to present their CSP membership card at time of check-in.

Members are responsible to notify the clinic in writing of their change of address or change of credit card information for electronic funds transfer (EFT) which are scheduled monthly. Simply go to the front desk and complete and return a Payment Authorization form.

You have the entire term of your membership to use your pre-paid chiropractic visits. Any unused visits will expire should you elect not to renew your membership.

Appointments

Scheduling & Booking Your Appointment

Booking your appointment has never been easier. Simply call the clinic anytime during normal business hours to schedule your appointment.

If you have a demanding schedule and are unsure when to book your appointment, set up a convenient standing appointment and you can come in on the same day and time at the frequency you desire.

Canceling Your Appointment

A minimum of 24 hours notice is required to cancel an appointment. Appointments cancelled with greater than 24 hours notice will not be charged a cancellation fee.

Appointments cancelled with less than 24 hours notice may be assessed a cancellation fee of 50% of the scheduled service upon the next visit.

Appointments that are not met due to a no call/ no show situation, may be assessed a 100% cancellation fee equal to the price of the scheduled service upon the next visit.

Guest Information

Massage Guests

Our member guest policy is simple: as long as your membership status is active and dues current, you may bring the same guest twice every 6 months, and your guest receives your member rate. A visit is defined as a 30, 60, or 90 minute massage at the member rate.

  • A massage guest must be accompanied by the member or present a guest pass at time of appointment.
  • After a massage guest's first visit, all subsequent visits are priced at non-member rates unless accompanied by a current member, presenting a guest pass or gift certificate.
  • Massage guests age 18 years old and above are considered adults and do not require special consent or parental/guardian supervision.
  • Massage guests age 15-17 years old must present written parental/guardian permission prior to a massage session.
  • Massage guests age 14 years old and under are required to be accompanied by a parental/guardian at all times while in the clinic.

Chiropractic Guests

Chiropractic guests are also welcome! As long as your membership is current a guest pass may be used only for an initial chiropractic examination (e.g., for a NEW patient), treatment and 30-minute massage for only $99.

Members in good standing are entitled to offer a chiropractic guest pass to a work associate, friend, acquaintance or loved one once per month for use at the time of a New Patient chiropractic visit. Age restrictions apply as with massage guests.

Gift Certificates

Gift certificates may be purchased by members anytime at member rates. A guest may also purchase gift certificates anytime at the non-member rate.

Gift certificates are non-refundable.
